GCRC Advisory Committee (GAC) Application Form Please use this form for catgory A and category B visits Please email your completed form to gcrcadmin@georgetown.edu and fax the signature sheet to 202.444.4114 or deliver it to GCRC administrative offices in M7201. As part of your application package, please also provide a copy of the full protocol, IRB application, informed consent form and/or assent form, study budget, and IRB approval letter (if available at the time of GAC submission). Clinical Trials Unit (CTU) Application Form Please use this form for Industry-sponsored studies (if not a rare disease study) Please email your completed form to gcrcadmin@georgetown.edu and fax the signature sheet to 202.444.4114 or deliver it to GCRC administrative offices in M7201. As part of your application package, please also provide a copy of the full protocol, IRB application, informed consent form and/or assent form, and IRB approval letter (if available at the time of CTU submission).
